(1) If a person who receives any social security allowance as per section 3 of the Act does not want to take such social security allowance voluntarily, he himself or his guardian, guardian or care giver can submit an application to the concerned ward office in the format as per schedule-4.
(2) If an application is received according to sub-rule (1), the ward office shall act on the same and remove the name of the person from the list of persons entitled to social security allowance.
(3) If the person whose name has been removed from the list according to sub-rule (2) wants to receive the security allowance again, he must complete the process according to this regulation and include his name and thus such allowance will be provided due to the fact that the name is included in the record again.
